The Atikokan Family Health Team (AFHT) is Your Partner in Everyday Health

We’re here to support you every step of the way by delivering comprehensive, compassionate care in collaboration with the Atikokan Medical Clinic to ensure your healthcare journey is a positive one. Our goal is to help you and your family stay well, manage chronic conditions, and access the services you need close to home.


Our team includes:

  • Registered Nurses (RNs)
  • Registered Practical Nurses (RPNs)
  • A Nurse Practitioner
  • A Dietitian
  • A Pharmacist
  • A Mental Health Worker


Together, we support patients and families through preventive care, health education, chronic disease management, mental health services, and coordinated care planning. We also collaborate with the Atikokan General Hospital and other healthcare partners to ensure continuity of care across the AHCS system and beyond.

Providing services in the community of Atikokan since 2005, the Atikokan Family Health Team is the primary care services arm of the amalgamated Atikokan Health and Community Services (AHCS) organization.


The Atikokan Family Health Team is governed by the AHCS Board of Directors, which is comprised of community volunteers, physician representation, and AHCS Senior Leadership, ensuring that the primary care needs of our community are met.
